When to arrive
On average, your first physical therapy appointment will last about an hour. Please arrive 15 minutes early to sign in, complete paperwork, and change clothes, if necessary.
What to bring
At your first physical therapy appointment, you’ll need to bring your physician referral or prescription (if applicable), your insurance card, registration forms, ID, and co-payment (if applicable). You may also bring a change of clothes.
How it works
During your appointment, your physical therapist will complete an initial evaluation and discuss your care plan. They’ll also work with you to make a plan for your continued treatment. Your physical therapy goals may include improved movement, strength, endurance, and flexibility, as well as decreased pain.

We work with most major insurance providers and do our best to help keep the paperwork pain-free. If you have questions about physical therapy insurance coverage, please ask when you schedule your appointment.
If your insurance provider requires a co-pay, we will ask for payment at each visit. We accept cash, check, and credit card.
